At the top of the envelope, there are two flaps to close the envelope. One has adhesive the other does not. The one with adhesive was folded into the envelope and stuck to the inside. This meant that the envelope was wide open when shipped. Predictably, the product fell out during shipping and the shipper delivered an empty envelope which was recorded as "delivered".
