  It said in their information there were 5 forms in their package. I received 2 forms and the main one I needed wasn't even there. If I had known what they sent me was their actual product I wouldn't have bought it. It wasn't what I needed and it was priced too high for what I got.

  i made this order on ebay from a seller back east and estimated arrival in about a week. The order came the next day delivered by an Amazon driver in Amazon packaging. This was very confusing because I did not order from Amazon. If this was normal procedure, someone should have informed me which did not happen. I have purchased several things in the past and never had a problem till now with something from ebay. Kind of disappointed!!
