Cap did not fit tightly on the nozzle and hence fuel emission from tank would not be contained.
Product did not fit tightly on the fuel tank nozzle.
It is highly probable that the product had a manufacturer's defect in the location of the tabs on the cap to prevent the cap from tighten.
