It was broken.
I did not know that open box meant damaged. The piece that covers the water tank was off and lying in the box. I found a tiny grey cylinder that looked like it was important so I saved it. Turns out this is part of the hinge which attaches one side of the door to the water reservoir. It is actually broken (as in snapped off). So in reality I would not have purchased this iron as the "New Open Box" description is misleading. It should be NEW DAMAGED OPEN BOX.
